1. **Build the form**

    Add **Short answer** (Name), **Email**, **Short answer** (Company), a
    **Dropdown** for *Team size* and a **Long answer** for *What do you need?*.
    Mark Name and Email as **Required**.

2. **Qualify leads with logic**

    Use [conditional logic](/logic/conditional-logic) to show a *Budget*
    question only when *Team size* is above your threshold.

3. **Score leads with AI (optional)**

    Add an [Autofield](/ai/autofields) called *Lead score (1–10, how ready to
    buy)* that reads the answers. Each lead gets a score automatically.

4. **Connect Notion**

    On the form's **Integrations** page, connect [Notion](/integrations/notion).
    Pick a page and let Finerlise create the database, or map answers to an
    existing CRM database. Add a *Status* property in Notion to track follow-up.

5. **Get notified**

    Turn on [email notifications](/settings/email-notifications) or send new
    leads to a [Discord channel](/integrations/discord).

6. **Publish and embed**

    [Embed](/share/embed-on-your-website) the form on your contact page, or open
    it as a [popup](/share/popup-forms) from a *Talk to sales* button.
