Every Finerlise form has a **Report this form** link at the bottom.

1. Click **Report this form**.
2. Choose why you're reporting it, such as phishing or impersonation.
3. Add any details that help us understand the problem and, optionally, your email so we can follow up.
4. Click **Submit report**.

We review every report. Forms that break our [acceptable use policy](/resources/acceptable-use) are taken down.
